Visiting physician support
Discuss the community’s clinical needs, planned visit cadence, communication routes, and coordination with each resident’s chosen clinicians.
Assisted living & memory care
Explore visiting physician care, follow-up after a hospital stay, and defined communication with wellness teams—guided by resident choice and existing care relationships.
Where we can help
Discuss the community’s clinical needs, planned visit cadence, communication routes, and coordination with each resident’s chosen clinicians.
Clarify follow-up needs, medication questions, and responsibility for next steps in partnership with the resident’s clinical and wellness teams.
Define how concerns reach the responsible clinician, how families receive updates, and what to do when the usual contact is unavailable.
Built around the resident
A community partnership does not automatically enroll every resident or replace a chosen physician. Resident choice, consent, clinical needs, and the agreed arrangement guide participation.
For memory care communities, we discuss communication with authorized family members or representatives and coordination with the existing clinical team.
Define the relationship
Use the scope discussion to agree on clinical responsibility, community coordination, and resident participation.
| Participant | Responsibility to clarify | What the plan should identify |
|---|---|---|
| Resident and chosen representatives | Preferences, participation, consent, and existing clinical relationships. | Who is involved in decisions and how preferences are communicated. |
| Responsible clinician | Clinical assessment and treatment within the established relationship. | Visits, follow-up responsibility, and contact arrangements. |
| Community and wellness team | Observation, community services, and coordination within the team’s role. | How concerns reach the clinician and how the response is shared. |
Before we begin
A useful first conversation starts with clear responsibilities, coverage expectations, and a workable path to launch.
Yes. Resident choice and existing clinical relationships guide participation. A community partnership does not automatically enroll every resident or replace a chosen physician. The scope discussion clarifies how any participating clinicians coordinate.
The community retains its wellness, nursing, and administrative responsibilities. The proposed physician role, participating residents, communication routes, and follow-up owners are defined together before work begins.
We agree on the visit cadence, contact route, response expectations, and backup arrangements for nights, weekends, and clinician absences. Those details depend on the facility and the contracted service; an inquiry does not establish round-the-clock coverage.
Medical-director and administrative services are defined in the facility agreement. BridgeCare handles billing for clinical services performed by its clinicians under the applicable payer or payment arrangement. Scope, fees, billing responsibilities, and coverage requirements are confirmed before work begins.
Start timing depends on the agreed scope, clinician availability and licensure, facility credentialing and onboarding, and any applicable payer requirements. We confirm a start date once the necessary arrangements are in place.
Region & availability
We welcome facility inquiries from Delaware, New Jersey, Pennsylvania, and Vermont. Availability and service scope are reviewed for each facility and depend on location, licensure, credentialing, schedule, and clinician capacity.
Community type, location, current physician model, broad support needs, and desired timing. No resident details are needed.
