Dr. Rishi Gulati combines his experience in post-acute and long-term care with fellowship training in critical care medicine.
His role at BridgeCare
Rishi leads the clinical approach and facility relationships. His work focuses on reassessment, direct communication with nursing, and conversations that connect treatment decisions with the resident’s goals.

Internal medicine training at Hackensack Meridian Ocean University Medical Center, including service as chief medical resident, and pulmonary and critical care fellowship training at Jefferson Health New Jersey. Qasim earned his osteopathic medical degree at Touro College of Osteopathic Medicine.

Qasim supports New Jersey operations with a clinical focus on attentive reassessment and changes in a resident’s condition.
Internal medicine training at Suburban Community Hospital and critical care training at Jefferson Washington Township Hospital, where Haroon served as chief fellow. His experience includes subacute rehabilitation, nursing homes, multidisciplinary rounds, and teaching residents and medical students.

Haroon brings his internal medicine, critical care, and post-acute experience to BridgeCare’s Vermont operations.
Experience in primary care, home-based care, assisted living, hospice, palliative care, and care transitions. Janet earned her Doctor of Nursing Practice from Chamberlain University and holds an MBA in Healthcare Administration and Marketing and an MSN in Nursing Leadership.

Janet focuses on patient advocacy, family education, and coordination with physicians, caregivers, and facility teams.

Documentation Operations Lead

Harold Von Navio
Billing & Credentialing Lead

Billing coordination, provider enrollment, and payer credentialing.

Harold leads application follow-up and coordinates the enrollment and billing work behind BridgeCare’s clinician operations.
Hospital relations, healthcare marketing, and admissions across rehabilitation, skilled nursing, and hospice settings. AnnaMaria is fluent in Italian and has professional experience interpreting for Italian-speaking patients.

AnnaMaria brings her experience with hospitals, families, and care teams to understanding facility needs and building community relationships.
Social work, behavioral health, case management, and community services. Janea holds a master’s degree in social work and has experience supporting patients and families, working with interdisciplinary teams, and connecting people with resources.

Hospice, home care, healthcare marketing, and community outreach. Jennie is a Certified Medical Interpreter, is bilingual in English and Spanish, and holds an associate degree in Business Administration–Accounting from Strayer University.
