BridgeCare Medicine is designed for non-emergency physician advisory support after serious illness, hospitalization, ICU care, discharge, or rehab transition.
Information We Collect
BridgeCare may collect basic contact and inquiry information such as name, email address, phone number, general reason for inquiry, referral source, scheduling information, and payment status.
Medical records or protected health information should be submitted only through an approved secure intake or upload workflow after fit, consent, and scope are confirmed.
Do Not Send Records By Ordinary Email
Do not send discharge summaries, medication lists, lab reports, images, dates of birth, medical record numbers, or urgent symptom updates through ordinary email, text message, social media, or a general website form.
How Information Is Used
Information may be used to respond to inquiries, determine fit, schedule advisory visits, process payment, provide advisory services, maintain records, and improve operations.
How Information May Be Shared
BridgeCare does not sell personal information. Information may be shared with vendors needed to operate the service, payment processors, secure intake or upload platforms, scheduling or video platforms, and professional advisors such as legal, accounting, or compliance support.
Medical information is shared with treating clinicians only when appropriate authorization or direction is in place.
SMS And Text Messaging
If you choose to communicate with BridgeCare Medicine by text message, BridgeCare may use SMS to respond to inquiries, coordinate scheduling, confirm callback details, and provide non-emergency administrative updates.
Do not send protected health information, medical records, medication lists, diagnoses, dates of birth, medical record numbers, urgent symptom updates, or emergency information by text message.
SMS consent is not shared with third parties or affiliates for marketing purposes. Message and data rates may apply. Message frequency varies. You may reply STOP to opt out or HELP for assistance.
Analytics And Advertising
Public website analytics or advertising tools should not receive protected health information, medical record details, diagnoses, medication information, patient identifiers, or form contents.
Emergency Boundary
BridgeCare is not emergency care. For urgent symptoms or time-sensitive concerns, call 911, contact the treating clinician, contact facility staff, or go to the emergency department.
BridgeCare does not replace the treating team, emergency services, or the clinicians already responsible for care.
